PHYSICAL EXAMINATIONS

Please call to make an appointment at our scheduled clinics.

If you are undergoing a comprehensive entrance physical examination, you must bring copies of your immunization records with you.

Note: If Revenants are not added to the current team roster, they will be charged a fee for their physical. Only if your name is on the center’s list will you be able to get a free medical examination.

FSC Sports Medicine will hold an ortho screening and physical date on Saturday July 30e. We would like everyone to be present on this date, if possible. We give you the option to do your physical exam on your own before then, so you can just come in for the ortho screening and to complete your basic concussion test. The health and wellness center will be open all summer to do physical exams on Wednesdays, Thursdays and Fridays, by appointment. All athletes can call to make an appointment, insurance is not necessary for these physics. We are looking to eliminate semi-annual clearances, so if you cannot get a physical with your primary physician between 6/1 and 8/1, please use Health and Wellness. Appointments can be made at 934-420-2009.

Please note: new athletes and transferring athletes will need copies of their vaccinations (MMR/Measles/Mumps/Rubella/Hepatitis B/Meningococcal (resident students only)/TDAP (within 10 years) as well as a verification of their sickle cell trait status.These forms can be found in the New Student/Transfer Athlete Documents

IF an athlete cannot make July 30e date, please email us at FSCSportsmedicine@farmingdale.edu to discuss options.

**This year all documentation including physical exams, vaccinations (new/transferring athletes only), Cardiac Questionnaire (new/transferring athletes only) and Meningococcal Immunization Response Form (new/transferring athletes only) transferred only), will be collected by the athletic training department. Personal. Athletes will no longer be required to submit it to Health in Wellness to be cleared.**

INSURANCE COVER

EVERYTHING athletes are required to have medical insurance. If they don’t, they can apply through the New York State Health Plan Marketplace.
